It is Brain Injury Awareness Month, and there is an important connection that is still widely misunderstood.
The connection between traumatic brain injury, altered brain chemistry, and substance use.
In many of the recovery and criminal justice settings where I work, people often say:
“Why do people with brain injuries self-medicate?”
But when we actually look at the neuroscience, the question becomes much clearer.
Traumatic brain injury commonly alters several key neurotransmitter systems, particularly:
- Dopamine — motivation, reward, drive
- GABA — calming and regulation
- Glutamate — learning and excitatory signaling
Research shows that brain injury can lead to:
- Reduced dopamine signaling
- Disruption of the GABA / glutamate balance
- Neuroinflammation
- Cognitive and emotional dysregulation
When these systems are disrupted, the brain struggles to regulate:
- stress
- emotion
- attention
- sensory input
No explanation.
No education.
Just confusion.
Many individuals with brain injury experience:
- sensory overload
- anxiety and agitation
- cognitive fatigue
- emotional instability
- difficulty focusing or regulating stress
And then something happens.
They discover that alcohol or drugs change how they feel.
Alcohol increases GABA activity, calming the nervous system.
Stimulants increase dopamine, temporarily restoring motivation and focus.
Other substances can dampen overwhelming sensory input.
From the perspective of the survival brain, this makes perfect sense.
The brain is simply trying to regulate itself with whatever tools it finds.
In that moment, the brain says:
“This works.”
And for a while, it does.
Substances can temporarily stabilize the dysregulated brain.
But over time, those same substances begin to further damage the systems they were compensating for.
What started as regulation becomes addiction and collapse.
This is why education about the brain matters.
When people finally understand what happened to their brain, something powerful often happens:
The shame begins to lift.
The behavior starts to make sense.
And recovery becomes possible.
As we continue raising awareness about brain injury, we must also recognize how often unrecognized brain injuries sit underneath addiction, mental health struggles, and involvement with the justice system.
Understanding the brain does not excuse harmful behavior.
But it can explain it.
And explanation is often the first step toward real change and recovery.
You cannot change what you do not understand.
Knowledge of the brain gives people the instruction manual they were never given.
